What is SCN?

Social Care Networks are part of New York's 1115 Medicaid waiver. Lehoel is your front door — a free, multilingual service for Medicaid members and their families.

Do I qualify?

If you're a New Yorker on Medicaid, the answer is almost certainly yes.

  • You live in a New York SCN region
  • You're already receiving some services — that's fine
  • Not sure? We'll check eligibility for free during your screening.

SCN services are funded by the New York State 1115 Medicaid waiver. Signing up will not affect your Medicaid, immigration status, or other benefits.

FAQ

Common questions

  • Yes. Lehoel is funded through New York's 1115 Medicaid waiver. There is no cost to you or your family for screening, navigation, or referrals.
  • No. SCN participation is a benefit of being on Medicaid — it doesn't change your coverage, your case, or any other program you're enrolled in.
  • No. We follow HIPAA-aligned confidentiality. Your information is shared only with the community partners you agree to be connected with, and never with immigration enforcement.
  • No. You can begin without any paperwork. Some specific services (like SNAP or housing applications) require documents later, and your navigator will walk you through what's needed.
  • SCN services are for Medicaid members, but we can still point you to community resources and help you check whether you qualify for Medicaid.
  • Yes — across the whole process: intake, screening, navigator calls, and follow-up. Choose your preferred language when you sign up.
  • The first screening is 15–20 minutes. Most members are matched with a partner within 1–2 business days, and many receive their first service within a week.
