SCN services are funded by the New York State 1115 Medicaid waiver. Signing up will not affect your Medicaid, immigration status, or other benefits.
We're a small, mission-driven team of navigators who do this work because we believe whole-person care starts with listening.
You talk to the same navigator each time. No call-center handoffs, no robo-questionnaires.
Every navigator speaks English. Most also work in Spanish or Haitian Creole.
Your information stays between you and your navigator. We only share what you ask us to.
